Appendix A. Checklist of Formatting and Filing Standards

Page Number Placement |
Page numbers should be centered, placed .75 inches from bottom edge of pages and consistently placed throughout the manuscript |

Pagination Standards |
Excepting title and approval pages, each page of manuscript must be numbered - no missing, blank, or duplicate numbers or pages; use lower case Roman numerals to number preliminary pages; use Arabic numerals to number text and back matter |

Numbering of Preliminary Pages |
Title and approval pages are counted but not numbered; subsequent pages (e.g. the abstract) are numbered beginning with Roman numeral iii |

One-sided Printing |
One-sided printing of all pages of the manuscript is required |

Spacing Between Lines |
Preliminary pages (except for title page) and text must be double-spaced; long quotations, footnotes, captions, tables, original surveys may be single-spaced (ProQuest requirement; see Chapter II for exceptions) |

Fonts & Font Sizes |
A font of at least 12 for preliminary pages and text; a font size of at least 10 for footnotes and captions; Use of standard font recommended |

Standards Governing Manuscript Titles |
Use concise manuscript titles (not to exceed 50 words) with easily identifiable key words that summarize research; use word substitutes in place of non-alphabetical symbols in scientific titles (ProQuest requirement) |

Faculty Signatures on Approval Pages |
All committee members must sign in black or blue ink |
|
Responsibility for Content |
Students and committee members are responsible for everything in the manuscript; committee members must review entire manuscript before signing |

Permission Letters |
Letters granting permission to use previously copyrighted material must accompany manuscript when copying exceeds fair use (ProQuest requirement) |

Copyrighting |
Registration of a copyright is optional; see Chapters III and IX |
|
Permission to Microfilm Required for Dissertations |
Doctoral candidates are required to give permission to ProQuest to reproduce and distribute the dissertation using microfilm and electronic means |

Doctoral Defense Forms |
Doctoral students must have appropriate form on file in Graduate Division indicating either that they defended the dissertation or had defense waived |

Students must be registered or pay a Filing Fee (one-half of Registration Fee) |
