Checklist of Items Required to File a Doctoral Dissertation
The Following Items are to be brought to the Graduate Division (3rd Floor Cheadle Hall):

Deliver 2 unbound copies of your dissertation on 100% cotton bond paper, 20-24 lb. (including 2 original signature pages on 100% cotton bond paper, 20-24 lb. and two extra copies of your title page and abstract)

If you are planning on electronically filing a copy of your dissertation then you will be responsible for bringing only one unbound copy of your dissertation on 100% cotton bond paper, 20-24 lb. to the Graduate Division (including 2 original signature pages also on 100% cotton bond paper and 1 extra copy of your title page and abstract) AFTER you file your dissertation electronically.

Form III or Form III-A -- Dissertation defense or waiver of formal defense form. Any changes to your committee must be noted on a Committee Form I-A
Completed and signed Doctoral Dissertation Publishing Agreement Form authorizing ProQuest to reproduce and distribute copies of your dissertation (required by Graduate Council). Students choosing to electronically file a copy of their dissertation will complete the ProQuest Publishing Agreement Form online when they file electronically.
Complete the Survey of Earned Doctorates (SED). Please download and print out the PDF version of the SED, fill it out and bring it with you to the Graduate Division when you file.
Completing the Doctoral Exit Survey online.
Optional: Payment of a $65.00 copyright fee to ProQuest to register the copyright of your dissertation with the Library of Congress Copyright Office.
Copyright privileges reside with you immediately upon creation of your work. Registration of your copyright establishes a public record of your dissertation and confers additional legal rights, enabling you to file infringement suits and seek statutory damages and attorneys' fees.
Students choosing to electronically file a copy of their dissertation will be able to pay the ProQuest copyright fee with credit card when electronically filing. For students choosing not to electronically file a copy of their dissertation this copyright fee is only payable by either a certified check or money order with an expiration date of at least 6 months from the date of filing.
You may also choose to pursue other means to secure copyright privileges.
